Stewart L, Pellegrini CA, Way LW. Cholangiooenous Reflux Pathways as Defined by Corrosion Casting and Scanning Electron Microscopy. The American Journal of Surgery 1988; 155: 23-8. Using corrosion casting and scanning electron microscopy of the rat biliary tree, we investigated the site and size of the pathways that allow bacteria to reflux from bile to blood. Nonobstructed rat biliary trees were injected retrograde with methylmethacrylate resin at a constant rate of 0.04 ml/min to volumes of 40, 60, 80, 120, 160, and HPB INTERNATIONAL 221 200 !. The infusion pressure was monitored and a pressure-volume curve was constructed. After polymerization and corrosion in 30 percent potassium hydroxide, the casts were examined with scanning electron microscopy. In addition, to identify the size of the reflux pathways, ceramic particles of 150 A, 1.7 , or 10 g were added to the resin, and the studies were repeated. Finally, intact livers with casted biliary trees were processed and studied by scanning electron microscopy without corro-

Frequency of Biliary Infection and Antimicrobial Susceptibility Pattern in Patients with Extra-Hepatic Biliary Obstruction Undergoing Non-Surgical Interventions with Reused Accessories

Bile is normally sterile. Bactibilia occurs when bile flow is impaired and bacteria gain access to the biliary duct by the papilla or portal circulation. Bile duct obstruction leads sequentially to increase in intra biliary pressure, infection of bile, cholangiovenous and cholangiolymphatic reflux leading to bacteremia [1-3]. The bacteria described commonly in biliary tract infections include E...

Antireflux Valve in Post Cholecystojeju-nostomy Cholangitis: An Animal Study

Background: One of the most important post-operation problems encountered with Hepaticojejunostomy (Kasai operation) in patients with biliary atresia is cholangitis. The aim of this study was to evaluate the efficiency of anti-reflux valve in decreasing the incidence of cholangitis after hepaticojejunostomy. Methods: Ten dogs were randomly divided into two groups of equal size. In group A Roux-...

Ascending Cholangitis due to Heavy Lifting

Gallstone disease is the most common risk factor for cholangitis. In an anatomically normal bile duct system, cholangitis does not occur without the presence of stones. Endoscopic retrograde cholangiography with papillotomy and stone extraction is a well-established curative therapy for gallstones in the common bile duct. More important, papillotomy prevents recurrent episodes. The present case...

Worsening of Acute Cholangitis Caused by a Bile Duct Stone After Hospitalization: A Case Series

Acute cholangitis is a fatal condition if inadequately treated. It is possible to underestimate the severity of the condition because bacterial cultures are not immediately available. We evaluated the clinical features of patients with cholangitis due to bile duct stones who were diagnosed with severe bacteremia at the time of hospitalization, but not at the time of the initial visit. We conduc...

A case of postoperative recurrent cholangitis after pancreaticoduodenectomy successfully treated by tract conversion surgery

A 69-year-old man, who had undergone pylorus-preserving pancreaticoduodenectomy (PD) (Imanaga procedure) for duodenum papilla cancer 13 years prior, had a history of repeated hospitalization due to cholangitis since the third year after surgery and liver abscess at the 10th year after surgery. Gastrointestinal series indicated no stenosis after the cholangiojejunostomy. However, reflux of contr...
